Subject Access Request

To see what data the National Disease Registration Service (NDRS) holds on you, you can make a Subject Access Request via the NHS Digital website.

If you would prefer not to send personal information by email, please provide an outline of the nature of your query or feedback along with the best way to contact you and a member of the team will be in touch shortly. Also, if you prefer to speak to a member of the team please provide us with your contact number below and we will call you shortly.

We will use the information to process your request and to provide any further relevant information or services you have requested. It will also be used to support our service development activities.
